Immunization Requirements
All students new to USF St. Petersburg are required to submit the following form and immunization documentation prior to being able to register for classes.
Students must complete and sign the Immunization Medical History Form then submit it HERE or mail it to:

An official translation is required for any forms not in the English language.
Immunization Medical History Form: According to Florida Administrative Code Rule 6C-6.001(5) “Each student accepted for admissions shall, prior to registration, submit on a form, provided by the institution, a medical history signed by the student.”
- Measles and Rubella Immunity (Required):
- Provide proof of two MMR vaccine dates (given after 1st birthday), or
- IgG quantitative lab report (performed within last 5 years). Lab report must include
result and reference range.
- Meningitis Immunity:
- Menactra/MCV4 vaccine (given after age 16) is an optional vaccine, however, it is strongly recommended, or
- Students can decline the vaccine through OASIS or by checking the box on the Immunization History Form.
- Hepatitis B Immunity:
- Hepatitis B is an optional vaccine, however, it is strongly recommended. Provide three (3) Hep B vaccine dates, or
- a quantitative titer blood test showing positive immunity, or
- decline the vaccine on the Immunization History Form or through OASIS
- TB Screening (Required):
- Skin Test (PPD or Mantoux), or
- Blood Test (Quantiferon gold/QFT or T-Spot), or
- Chest X-ray
- Per USF Policy 33-003, Tuberculosis Screening is required for all international & U.S. Born students residing at an address outside the U.S. Screening must be done within 6 months prior to the 1st semester you physically attend classes on any of the USF campuses.
If you are from Florida and your healthcare provider is a participating physician, you can log in to Florida Shots to receive a copy of your Immunization Records to submit with your Medical History form.
